Nguyên tắc MGH:
Không làm xét nghiệm thường quy – chỉ làm khi có chỉ định lâm sàng
4.1. Xét nghiệm thường cân nhắc:
- CBC
- Điện giải
- Creatinine
- HbA1c (đái tháo đường)
4.2. Cận lâm sàng:
- ECG: > 50 tuổi hoặc bệnh tim
- X-quang phổi: có bệnh phổi
- Echo: khi nghi suy tim/valve
5. Tối ưu hóa trước mổ (Optimization)
5.1. Tim mạch
- Kiểm soát HA
- Trì hoãn nếu ACS, suy tim mất bù
5.2. Hô hấp
- Ngừng hút thuốc
- Điều trị nhiễm trùng
5.3. Nội tiết
- Điều chỉnh đường huyết
5.4. Kháng đông
- Bridging nếu cần
6. Quản lý thuốc trước mổ
Nguyên tắc:
- Tiếp tục: beta-blocker, statin
- Ngưng/điều chỉnh:
- ACEi/ARB (tùy trường hợp)
- Kháng đông
- Thuốc hạ đường
7. Nhịn ăn trước mổ (NPO)
MGH (phù hợp ASA):
- Thức ăn đặc: ≥6–8 giờ
- Sữa: ≥ 6 giờ
- Dịch trong: ≥2 giờ
8. Lập kế hoạch gây mê (Anesthetic Plan)
