Quản lý Gây mê trong Phẫu thuật TURP: Cập nhật mới nhất theo Clinical Anesthesia 2025
Phẫu thuật cắt u phì đại lành tính tuyến tiền liệt qua đường niệu đạo (Transurethral Resection of the Prostate – TURP) vẫn được coi là “tiêu chuẩn vàng” trong điều trị ngoại khoa BPH. Tuy nhiên, đây là loại phẫu thuật tiềm ẩn nhiều thách thức về gây mê do đối tượng bệnh nhân thường là người cao tuổi có nhiều bệnh lý nền và các biến chứng đặc thù như Hội chứng nội soi (TURP Syndrome).
1. Đặc điểm Bệnh nhân và Đánh giá Tiền mê
Bệnh nhân TURP thường nằm trong độ tuổi từ 60 – 90, đối mặt với các nguy cơ:

Hệ tim mạch:Tăng huyết áp, bệnh mạch vành, suy tim.

Hệ hô hấp: COPD hoặc giảm dự trữ hô hấp do tuổi tác.
-
Hệ tiết niệu: Chức năng thận có thể suy giảm do tắc nghẽn mãn tính.
Lưu ý từ Barash 2025: Cần tối ưu hóa các bệnh lý nội khoa và đánh giá kỹ khả năng chịu đựng tư thế phụ khoa (lithotomy) – tư thế có thể làm thay đổi đáng kể cung lượng tim và thông khí phổi.
2. Lựa chọn Phương pháp Vô cảm: Tê tủy sống hay Gây mê nội khí quản?
Theo Clinical Anesthesia 10th, mặc dù cả hai phương pháp đều khả thi, Tê tủy sống (Spinal Anesthesia) thường được ưu tiên hơn vì các lý do sau:
Ưu điểm của Tê tủy sống:

Theo dõi tri giác: Bệnh nhân tỉnh táo giúp phát hiện sớm các dấu hiệu sớm của Hội chứng TURP (lú lẫn, kích thích) hoặc thủng bàng quang.

Giảm mất máu: Nhờ tác dụng giãn mạch hệ thống và giảm áp lực tĩnh mạch vùng chậu.
-
Giảm nguy cơ huyết khối: Cải thiện dòng máu tĩnh mạch chi dưới.

Kiểm soát đau sau mổ: Hiệu quả hơn và giảm nhu cầu sử dụng Opioid.
Thông số kỹ thuật: Mức phong bế cảm giác cần đạt đến T10 (tương ứng vùng rốn) để ức chế cảm giác đau từ bàng quang và niệu đạo.
3. Hội chứng TURP (TURP Syndrome): Biến chứng nguy hiểm nhất
Đây là tình trạng rối loạn thể tích và điện giải do hấp thu quá mức dịch rửa (thường là Glycine 1.5% hoặc Sorbitol) vào tuần hoàn.
Cơ chế và Triệu chứng:

Quá tải tuần hoàn: Gây tăng huyết áp, nhịp chậm, phù phổi cấp.
-
Hạ Natri máu (Hyponatremia): Khi Na+ < 120 mEq/L, bệnh nhân xuất hiện co giật, hôn mê.
-
Ngộ độc Glycine: Glycine chuyển hóa thành Amoniac gây độc thần kinh và có thể gây mù tạm thời (do tác dụng ức chế dẫn truyền tại võng mạc).
Quản lý cấp cứu:

Dừng phẫu thuật càng sớm càng tốt.
-
Hỗ trợ hô hấp, tuần hoàn.
-
Lợi tiểu: Furosemid để thải dịch dư thừa.
-
Natri Clorua ưu trương (3%): Chỉ dùng khi có triệu chứng thần kinh nặng, tốc độ bù không quá 1-2 mEq/L/h để tránh hội chứng hủy Myelin trung tâm cầu não.
4. Các biến chứng khác cần lưu ý
Thủng bàng quang (Bladder Perforation)
Thường xảy ra do diện cắt quá sâu hoặc cử động bất thường của bệnh nhân (phản xạ thần kinh bịt).

Triệu chứng (khi tê tủy sống): Đau bụng cấp tính, đau vai (do kích thích cơ hoành), bụng chướng.
-
Xử trí: Đặt dẫn lưu hoặc can thiệp ngoại khoa nếu thủng phúc mạc.
Hạ thân nhiệt
Dịch rửa lạnh với số lượng lớn có thể làm giảm nhiệt độ trung tâm nhanh chóng.

Giải pháp: Làm ấm dịch rửa lên 37°C và sử dụng máy sưởi ấm cho bệnh nhân.
5. Theo dõi và Chăm sóc Hậu phẫu

Theo dõi sát điện giải: Đặc biệt là Natri và Hematocrit ngay sau mổ.

Kiểm soát co thắt bàng quang: Sử dụng các thuốc giảm co thắt hoặc tê ngoài màng cứng phối hợp.
-
Vận động sớm: Ngăn ngừa biến chứng thuyên tắc mạch.
